Privacy Policy

Privacy and protection of personal information, including personal health information is important to HouseCalls Physiotherapy (HCP). We are committed to collecting, using and disclosing personal information responsibly, and only to the extent necessary for the services we provide. HCP is a home visit physiotherapy treatment provider that is physiotherapist owned and operated. In addition to the owner, contract providers include another physiotherapist, a registered kinesiologist, and physiotherapist assistants. The Privacy Officer for HCP is the owner Mark Peever. The duties of the Privacy Officer include reviewing and implementing any changes in policies and procedures to insure HCP meets its obligations under and remains compliant with the Personal Information and Electronic Documents Act (PIPEDA) and Personal Health Information Protection Act (PIHIPA). He is responsible for informing clients, contractors and the public about HCPs privacy policy relating to collection, use and disclosure of personal and personal health information, and for addressing any complaints associated with privacy of information. Finally, he is responsible for informing individuals in the event that their personal information is lost or improperly disclosed. HCP collects information by phone, fax, email, personal interview, examination, and from other health providers or stakeholders. Information is collected for the purpose of delivering safe and effective treatment, to document changes that occur over time, to comply with regulatory requirements, for client and family contact, for communication with other health providers and stakeholders involved in your care and for invoicing. Information may be shared with other health providers involved in your circle of care by implied consent. Express written consent is obtained prior to sharing information with other stakeholders such as insurance adjusters or lawyers. Personal information is stored in paper and electronic format. Paper information is kept secure by supervision or in a locked cabinet. Electronic information is password protected and encrypted. We use an online platform for collection and storage of billing information which follows its own standards for information protection. Health information is retained for 10 years after the last entry for adults, and 10 years after the 18th birthday for children. Clients may access their health record by written request. Corrections or amendments to factual content of the record can be made on request. Please contact the above named Privacy Officer for any questions or concerns regarding personal information.
