Assistive Devices Program Application for walkers: $90.00 at the store. $120 at your home (includes mobility assessment and completion of ADP form). Wheelchairs: $480 – includes initial assessment, form completion, trial visit and dispense visit. Additional visits, if required, are billed at regular rate of $120 per visit. Travel surcharge may be applied depending on area (see list below).
These rates apply for the following areas: Arnprior, White Lake, Burnstown, Renfrew, Cobden, Foresters Falls, Beachburg, Westmeath, Lapasse, Pembroke, Douglas, Osceola, Micksburg, Eganville, Golden Lake and Petawawa. The rate for Calabogie or Killaloe is $140 per PT visit. For home or site visits within Renfrew County, but beyond the areas described above, please call for an estimate. The hourly PT rate for non-catastrophic MVA injuries is $99.75 including travel time. The hourly PTA rate is $58.19 including travel. The rate for catastrophic MVA injuries is $119.92/hr for PT and $89.07/hr for PTA. HouseCalls Physiotherapy is licensed by the Financial Services Regulatory Authority of Ontario, so visits covered by motor vehicle insurance are billed directly through Health Claims for Auto Insurance (HCAI). Visits covered by extended health benefit plans/private health insurance are billed to the client, or submitted through Telus Health if assigned by the client. We are registered for direct billing for Veteran’s (VAC) and WSIB. We accept payment by cash, cheque, interac e-payment, or credit (Visa/MasterCard).
A fee of $60.00 for the physiotherapist and $40.00 for the physiotherapist’s assistant will be applied if a visit is cancelled with less than 24 hours advanced notice. The full visit fee is charged for same day cancellations or if the client is unavailable when the therapist arrives.
